If you are interested in attending the online surgery taking place on Friday, 15th May, please complete the Surgery Sign-Up Form available for download at the bottom of this page and return it to my office at angela.richardson.mp@parliament.uk. If you wish to post a hard copy, please follow the instructions at the top of the form.
Once my office has received the form, a written confirmation of the date and time of your appointment will be e-mailed to you. Please note that appointments last for 20 minutes and slots are being allocated on a first come, first served basis, between 10-12 am.
To electronically fill out the form, click on the title below. This will open it in your web browser. Click on the Download icon in the top right to save it to your computer. Once saved, open the form in Adobe Acrobat Reader, put your details in the relevant fields, then click on the save icon, in the top left. Save the new file on your computer with a new name.
